Instructions
ALL FORMS MUST BE COMPLETED
- Power of attorney must be notarized and have your date of birth. We must have the original, no copies.
- The person who completes the power of attorney must be the same as the person whose name is to appear on the surety bond. If sending multiple transactions in different names, a power of attorney must be filled out separately for each name or business. Power of attorney will be kept on file and can be used to process any future transactions under the business or individual name.
- The serial (VIN) number must be legible.
- We need a VIN verification for signed and dated by your local police department, motor vehicle department or any other authorized agent or business in your state.
- Bill of Sale showing your name, year, make, serial (VIN) number, price, signature and address of the seller. Bill of Sale must be notarized, no copies. You cannot use our form.
- Must have a photocopy of Drivers License.
- Check or money order must be payable to International Title Service. (if paying by credit card, see instructions below)
Credit Card Payments must include the Credit Card Number (Visa, Discover, JCB, MasterCard, American Express), Expiration Date, Name (as it appears on the credit card) and your phone number.

Bonded Title Form | Power of Attorney
International Title Service is assisting yo in securing a bonded title, you will be solely responsible for knowing the specific statutes, ules, regulations and/or other requirements for issuance of a bonded title in the state in which you are located. Internetaional Title Service assumes no responsibility for state-to-state differences in conditions and procedures for the issuance of bonded titles. It will therefor be your sole responsibility to secure all relevant information from your local tag & title agency, department of motor vehicles or courthouse to be sure that your application complies in all respects with your state laws.
